Monitoring the Patient During and After Zofran Administration

Continuous Monitoring: Observe the patient closely for at least one hour after Zofran administration. Monitor vital signs, including heart rate, blood pressure, and respiratory rate, every 15 minutes initially, then less frequently as the patient’s condition stabilizes.

Cardiac Rhythm: Pay close attention to the patient’s electrocardiogram (ECG) for any changes in heart rhythm, especially bradycardia. Report any abnormalities immediately.

Neurological Assessment: Regularly assess the patient’s level of consciousness, alertness, and orientation. Note any changes in mental status, such as confusion or drowsiness. Check for any signs of extrapyramidal symptoms, like tremors or rigidity.

Hydration Status: Assess the patient’s hydration status throughout the monitoring period. Zofran can cause dry mouth; encourage fluid intake.

Adverse Reactions: Watch for symptoms of adverse reactions, including headache, dizziness, and constipation. Document any observed symptoms thoroughly.

Post-Administration Observation: Continue monitoring the patient for several hours post-infusion, adjusting the frequency based on their response. Provide education about potential side effects and what to report.

Documentation: Meticulously document all observations, interventions, and patient responses in the medical record. Include timing of administration and all relevant data points.
