Interactions with Other Medications and Substances

Doxepin can interact with many medications, so always inform your doctor and pharmacist of all medications, vitamins, and supplements you are taking. This includes over-the-counter drugs. Certain combinations can increase the risk of side effects or reduce the effectiveness of doxepin.

Central Nervous System Depressants

Combining doxepin with other central nervous system depressants, such as alcohol, opioids, benzodiazepines, or barbiturates, significantly increases the risk of drowsiness, sedation, and respiratory depression. Avoid concurrent use whenever possible. If unavoidable, your doctor will carefully monitor you for adverse effects and may adjust your dosage.

Anticholinergic Medications

Doxepin possesses anticholinergic properties. Concurrent use with other anticholinergic drugs (like some antihistamines, antidepressants, or anti-Parkinson’s medications) can worsen anticholinergic side effects, including dry mouth, constipation, blurred vision, and urinary retention. Your doctor may need to adjust your medication regimen.

Monoamine Oxidase Inhibitors (MAOIs)

Do not take doxepin with MAOIs. This combination can cause a potentially dangerous interaction, leading to serious complications. A sufficient time interval (usually at least 2 weeks) must elapse after stopping an MAOI before starting doxepin.

Other Important Interactions

Doxepin may interact with certain blood thinners, increasing the risk of bleeding. Similarly, it can affect the metabolism of some other medications, changing their effectiveness.
