Diltiazem can interact with several medications. Grapefruit juice significantly increases diltiazem levels; avoid it. Beta-blockers, when combined, may excessively lower heart rate. Monitor your blood pressure and pulse closely. Digoxin levels can rise with diltiazem use; regular blood tests are needed to maintain safe levels.

Simultaneous use with certain calcium channel blockers (like verapamil) increases the risk of dangerously low blood pressure and heart rate.
